Medical Coverage

Dental

  • Two dental options are offered
  • Preventive care is provided at 100% coverage with no deductible on both plans
  • Coverage available to benefits-eligible associates and their dependents
  • Coverage begins on the first day of active employment
  • Orthodontia is covered up to $1,000 (lifetime) for dependents up to age 19 with high option plan

Vision

  • Voluntary benefit that covers eye exams, lenses or contacts every 12 months and frames every 24 months
  • Coverage available to benefits-eligible associates and their dependents

Short-term Disability (STD) Salary Continuation

  • Available to eligible associates after 90 days of active employment
  • Coverage provided at no cost to the associate
  • Temporarily disabled associates may receive up to 100% of base pay, for a maximum benefit duration of 12 weeks

Long-term Disability (LTD) Insurance

  • Automatic enrollment for benefits eligible associates after 90 days of employment
  • Coverage provided at no cost to the associate through a salary gross up
  • Benefit is 60% of base salary

Cancer and Specified Disease Insurance

  • Benefits are paid directly to associate
  • Annual wellness benefit offered
  • Coverage available to benefits eligible associates and their dependents

Long-term Care Insurance

  • Guaranteed issue coverage for benefits eligible associates as at initial eligibility
  • Coverage is portable
  • Coverage is also available to dependents and extended family members such as parents and grandparents (Evidence of Insurability required)
